Follow the steps below to become a new patient
Step 1- Pre-reg & Intake Paperwork
Click the pre-registration link button to bring up the patient pre-registration page in a new window. Once finished return to this page and move on to New Patient Intake Paperwork.
Click the New Patient Intake Paperwork button to open the forms in a new window. When finished make sure you select send to send paperwork to the Patient Intake Dept.
Step 2- Call to schedule Appointment once intake has confirmed insurance
Once you have completed Pre-Registration and New Patient Intake Paperwork, the Patient Intake Department will verify insurance coverage and request records from your previous primary care provider. The Patient Intake Dept. will send an automated message to the phone number you registered with informing you that the intake process is complete and you may call to schedule your appointment.
Step 3- After you have scheduled your appointment, completing health questionnaire.
Once your appointment has been made, your patient portal will be activated. Using the email you provided in your intake and the password Welcome1! , you will log in to your patient portal to complete the patient health questionnaire. The questionnaire needs to be completed before your appointment. It may be possible to come in 30-60 minutes before your appointment to complete the questionnaire on the patient kiosk. However, you should allow at least 30 mins for this process as in addition to the questionnaire you will also be digitally signing forms and scanning in your insurance card and ID.
